History

• Male, 71 years.
• The patient reported longstanding tremulousness but a more troublesome tremor in the left arm and leg for four years.
• He had a family history of essential tremor affecting his father, three paternal aunts and two sisters.
• His tremor improves with alcohol.
• Propranolol treatment caused lightheadedness.
• His occupation had been as a labourer.
