Diagnosis
The patient was diagnosed as idiopathic Parkinson’s disease based on the clearly abnormal scan. The longstanding tremor, tremulous writing, improvement with alcohol and family history indicated coexistent essential tremor.

Management
Selegiline 5mg mane increased to bd with symptomatic response.
