Case study 4

Examination findings

  • The patient exhibited reduced blinking.
  • Her frontal reflexes were negative.
  • She had a rest tremor in both arms/hands which was greater on the right than the left, but of similar frequency.
  • She also showed postural and action tremor of the hands and arms of 8Hz which again was greater on the right than the left.
  • She exhibited a stooped posture and had a slight decrease in arm swing when walking which was more marked on the right than the left.
  • She had right wrist rigidity when activated.
  • She recovered on the pull test.
  • The symptoms were mild but generally suggestive of Parkinson’s disease, but due to some confounding features a scan using DaTSCAN was performed.
